VOLUNTEER WAIVER FORM

**EACH VOLUNTEER MUST COMPLETE THEIR OWN WAIVER**

This Release and Waiver of Liability ("Waiver") entered into by the undersigned volunteer ("Volunteer") releases Project Never Again, Inc. ("Project Never Again"), a non-profit entity organized and existing under Section 501(c)(3) of the United States Internal Revenue Code and the laws of the State of Oregon, and each of its directors, officers, and agents.  The Volunteer desires to provide volunteer services for Project Never Again and engage in activities related to serving as a volunteer.

WAIVER AND RELEASE:  I, the Volunteer, release and forever discharge and hold harmless Project Never Again from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services I provide while volunteering for Project Never Again.  I understand and acknowledge that this Waiver discharges Project Never Again from any liability or claim that I may have with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I am providing while volunteering for Project Never Again.

INSURANCE: I, the Volunteer, understand that Project Never Again is under no obligation to provide me with any special health, medical, or disability insurance coverage.  As such, I expressly waive any claim for compensation or liability on the part of Project Never Again.

MEDICAL TREATMENT:  I, the Volunteer, release and forever discharge Project Never Again from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with the services I provide while volunteering for Project Never Again.

ASSUMPTION OF RISK:  I, the Volunteer, understand that the services I provide while volunteering for Project Never Again may include activities that may be hazardous to me including, but not limited to the moving of heavy boxes, the use of sharp objects, and constant bending and turning of the body.  I hereby expressly and specifically assume the risk of injury or harm in performing these activities and release Project Never Again from all liability, illness, death, or property damage resulting from these activities while volunteering for Project Never Again.

PHOTOGRAPHIC RELEASE:  I, the Volunteer, grant and convey to Project Never Again all right, title, and interests in any and all photographs, images, video, or audio recordings in connection with my providing volunteer services for Project Never Again.

By typing my name in the appropriate fields below, I hereby express my understanding and willingly agree to the terms and conditions of this Release and Waiver of Liability as the undersigned volunteer.
